SCHOOL HEALTH:

School Immunization Requirements

Be sure your child is ready for kindergarten & 7th grade. 
If your child is turning five, has recently turned five, or is entering 7th grade, the school clinic will need updated immunization records. 

If your child requires medication during the school day, the Medication Authorization Form must be completed by parent/guardian and prescribing doctor.  Please see your school nurse for more information.
